This questionnaire is copyrighted mostly to prevent unapproved changes to it. Its use for individual purposes (e.g. supporting clinical assessment at a patient consultation) is free and does not require any authorisation.
Independent research: The use of the questionnaire by any research group to aggregate data from different patients requires communication with the CARAT group.
For-profit organisation and research: For-profit organisations or the use of the questionnaire within commercial/marketing activities will require case-by-case authorisation from the CARAT group.
